The Region IV Low Incidence Project supports one full-time audiologist to assist the local educational team with identifying students who may qualify for D/HH services.

Service Area: Alexandria, Ashby, Barnesville, Battle Lake, Brandon-Evansville, Breckenridge, Browns Valley, Campbell-Tintah, Chokio-Alberta, Clinton-Graceville-Beardsley, Detroit Lakes, Dilworth-Glyndon-Felton, Fergus Falls, Frazee, Hancock, Hawley, Herman-Norcross, Lake Park-Audubon, Minnewaska, Moorhead, Morris, New York Mills, Norman County East, Osakis, Parkers Prairie, Pelican Rapids, Perham, Rothsay, Ulen-Hitterdal, Underwood, West Central Area, and Wheaton
 
Testing locations: Alexandria, Fergus Falls, and Moorhead
 
What to expect: Age-appropriate testing options are available for students ranging in age from six months through high school graduation. Testing takes about 45 minutes; both headphone and insert earphone styles are available for testing.

Initial Referrals - Initial referrals for hearing evaluation are required from the school district.

Reevaluation - Use this form for all requests for student reevaluations.

 

 

Midwest Special Instruments

Lakes Country Service Cooperative will once again serve as a host site for Annual Calibration of audiometers, tympanometers, and otoacoustic emissions (OAE) screening equipment; with Midwest Special Instruments being the service provider.
